Description: 

CommunityCARE Home Hospice is a visiting hospice that offers personal care, emotional and spiritual support, respite care, transportation to appointments, bereavement support for individuals and groups, coordination of services, and funeral services. This hospice is a service of the Metropolitan Community Church of Toronto.
