Caring to the end of life is a website for patients, caregivers and health care professionals alike who need information about palliative care.
Making the transition to palliative care can be difficult and confusing. Patients and their families and caregivers will have lots of questions, and may find themselves with many new needs. Caring to the end of life provides information about palliative care, community resources and services for patients and their families, and tools to help cope with the experience of palliative care, all in once place. Now patients and caregivers can browse this information at their own pace, and even keep a record of what they have looked at in their online Care Journal. Health care professionals who are looking for answers to patients' questions and ways to meet their changing needs will also find lots of helpful information here.
