Members of the Palliative Care Team

Your palliative care team will be interdisciplinary. This means there will be professionals with all kinds of different skills and specialties working with you to make sure you get the best care possible.

 

There will always be a Physician on your care team. Either your family doctor, a home and palliative care physician, or a doctor from the hospital will be in charge of your care. Depending on the care model in your area, your family doctor or palliative care physician may work together with the hospital doctors. Nurses and Volunteers will be important members of your care team, and Personal Support Workers (PSWs) may come to your home to help give you care there.


Other people who may be on your care team are:

  • Social Worker
  • Pharmacist
  • Physiotherapist
  • Occupational therapist
  • Dietician
  • Discharge Planner/Home Care Coordinator
  • Speech Language Pathologist
  • Chaplain
  • Volunteers
